- Hospice nurses, social workers, chaplains, home health aides and volunteer visits to the patient at their residence.
- An on-call hospice nurse, 24-hours a day.
- Medications for pain and symptom management related to the terminal illness.
- Durable medical equipment and supplies.
- Counseling and spiritual support for the patient and family.
- Respite care for the caregiver.
- Anticipatory grief support for all family, friends and loved ones.
How do I qualify for services?
All patients with a life-limiting or terminal illness qualify for services if:
- Their primary care physician certifies a prognosis or life expectancy of 6 months or less.
- They have completed and are no longer receiving curative therapies or treatments.
Where is Care Provided?
Care may be provided in the home, a skilled nursing facility, or residential care home, also known as an assisted living facility.
What is the Cost of Care?
Care is generally covered under one of the following: Medicare hospice benefit, most insurance plans, HMO/PPO/Private and other managed care organizations.
