Qualifications for Skilled Home Healthcare

  • Medical necessity for the skilled services of a nurse or therapist
  • Physician’s order
  • Meet Medicare’s definition of homebound status (leaving home requires a taxing effort)

Specialty Programs

Skilled Nursing

We assess, educate and treat all aspects of:

  • medication, disease and pain management
  • assistance with IV infusion
  • cardiopulmonary assessment
  • wound, ostomy and catheter care

Speech Therapy

Our Speech-Language Pathologists (SLPs) are trained to evaluate and diagnose speech, language, cognitive-communication and swallowing disorders.  The services we provide may include:

  • improving strength, coordination and sensation for safe eating and swallowing
  • improving speech intelligibility
  • improving word recall and use of language
  • improving ability to understand what is heard
  • improving reasoning and memory and/or providing strategies to overcome memory deficits 

Medical Social Work

Our medical social worker is available to assist patients in locating community resources. Counseling and emotional support can be provided for patients and family as needed.

Physical Therapy

Our physical therapists provide education and treatment which increases activity and enhances personal independence. This includes:

  • specialized post-operative joint replacement therapy
  • home safety evaluation
  • assessment and improvement of strength, mobility and balance
  • transfer and gait training
  • fall prevention

Occupational Therapy

Our occupational therapists assist and teach new strategies and techniques for patients to become as independent as possible. This includes:

  • improve self-care skills
  • improve breathing techniques
  • caregiver training
  • evaluate safety equipment needs
  • increase upper body strength and function

Health Aide Services

Home health aides may provide assistance with bathing, grooming and dressing.
